Deficiency - 002 - Certificate of Official Capacity

Explanation: Certificate of Official Capacity is required.

Instructions: You have indicated that this claim is being made on behalf of the injured party or his/her estate. The Trust requires documentation of the representative's authority to act on behalf of the injured party or the injured party's estate. Such documentation might include Powers of Attorney appointing the representative to act on behalf of the injured party in pursuing a claim for asbestos injuries or, where the injured party is deceased, Letters Testamentary or Letters of Administration from a court appointing the representative as executor or administrator of the claimant's estate. This does not include birth certificates, marriage certificates, or Last Will and Testament.
