The chest x-ray or CT scan must be read by a Qualified Physician.
Detail Text: The chest x-ray, CT scan, or B-reader report submitted is not for the injured party of this claim. Please submit a report for the injured party based on the review of a chest x-ray, CT scan, or a B-reader report indicating the alleged injury. The chest x-ray or CT scan must be read by a board-certified physician (defined in Footnote 3 of TDP). The report must be dated and signed by the radiologist or physician, and must include information identifying the injured party.
