Instructions: Severe Asbestosis (Disease Level IV) requires either an ILO of 2/1 or greater or pathologic evidence of asbestosis. The most recent ILO reading and/or pathology report does not meet these requirements. Please submit either a B-reading of a more recent chest x-ray or a pathology report that provides acceptable findings for asbestosis. The B-reading and/or pathology report must meet the requirements as outlined in the TDP.
